What this document is
Solicitud de reconocimiento del grado de discapacidad is the Junta de Andalucía application (Código de procedimiento: 69). With it, the Assessment and Guidance Centre (Centro de Valoración y Orientación, CVO) examines the person and recognises a disability degree as a percentage.
A recognised degree of 33% or more gives access to services and benefits for people with disabilities. The procedure is governed by Real Decreto 888/2022 and Andalusian Decreto 255/2021; the form header still cites the former RD 1971/1999, but the procedure page lists this very form as the current one.
Who applies and where
- the person concerned or a representative applies: a parent or guardian of a minor, a court-appointed guardian, or a de facto guardian (guardador de hecho);
- anyone registered as resident in Andalusia is entitled to the assessment, as is anyone there in a situation of urgent personal, family or social need;
- the application is addressed to the CVO of your province; it can be lodged at any public registry office, by post (Correos) or online on the Junta portal; booking an appointment (cita previa) is recommended for Junta registry offices;
- according to the procedure catalogue, the decision takes up to 6 months; with no answer, administrative silence is negative;
- a review for worsening or improvement can be requested no earlier than two years after the decision, unless there was a diagnostic error or a substantial change of circumstances.
What the form asks
- the reason: initial assessment or review;
- the applicant's details: surnames and name, sex, DNI/NIE, date of birth, nationality, NUHSA (for a child without a DNI), address, phones and e-mail;
- the representative's details and address, if a representative applies;
- the type of disability (physical, mental, sensory) and any degree recognised before;
- how to receive notifications, objections to data checks, consent to checks of income tax (IRPF), health records (DIRAYA) and early-intervention data (ALBORADA);
- the list of attached documents, place and date of signing, and the CVO the application is addressed to.
What to bring
- copies of medical and psychological reports not in the SAS electronic health record (DIRAYA): private clinics, doctors outside Andalusia;
- proof of representation or de facto guardianship, if a representative applies;
- for a minor, a copy of the libro de familia; if the parents are separated or divorced, both sign unless either has lost parental authority;
- for a foreign national, a copy of the foreigner identity card (TIE) or, failing that, of the asylum application.
