What this document is
«Declaración responsable de progenitor» — Impreso M001 of Zaragoza town hall. The parent declares on their own responsibility that both parents keep custody of the children, there is no court ruling on custody, the other parent's signature cannot be obtained and the children live with them, and that they have sufficient legal capacity for the registration.
It accompanies the main registration sheet (hoja padronal): the declaration lists up to three children registered, or whose address is changed, with that sheet.
Who files it and where
- one parent, when the child is registered only with them, there is no custody ruling and no signature from the other parent;
- if both parents sign, the joint declaration M003 is filed instead; if there is a court ruling on custody, the town hall has other forms (M002 for sole custody, M002 bis) — check on zaragoza.es which one applies;
- filed together with the hoja padronal and documents at the town hall office that handles the padrón; check opening hours and appointments on zaragoza.es.
What the form asks
- parent: surnames and given name, DNI, NIE or passport;
- registration address in Zaragoza on one line;
- up to three children: surnames and given name, document (if any), date of birth;
- date of signing; the signature is handwritten.
What to bring
- the completed and signed declaration M001 and the hoja padronal;
- the parent's document: Spaniards — DNI or passport; EU citizens — NIE or residence card, passport or national ID card; other foreigners — NIE or residence card, or passport;
- for the children — Libro de Familia or birth certificate; from age 14 — DNI, NIE or passport;
- documents as original and copy; they must be valid;
- in cases of gender-based violence — the court decision, restraining order or police report;
- the town hall may ask for additional documents.
