What this document is
The "Hoja de inscripción o modificación" is the official form of Alicante City Council (Ayuntamiento de Alicante) for the municipal register of residents (Padrón Municipal de Habitantes): first registration, moving in from another town or country, a change of address within Alicante, the birth of a child or a correction of personal data.
The form is in Spanish, two landscape A4 pages: the first is filled in (up to four people at one address), the second holds the instructions, the list of documents and the education codes. FoxDocs only types your data into the fields of the official PDF and does not change its layout.
Who files it and where
- anyone living in Alicante, foreigners included; every adult listed on the sheet signs it;
- the council's procedure page marks the form as meant for the Sede Electrónica: you fill it in, sign it and attach it to the online application (an electronic signature is required — a digital certificate or DNIe);
- in person — at the Oficinas de Atención Ciudadana "Labradores" (C/ Labradores, 14) and "Séneca" (C/ Portugal, 17) by appointment: phone 010 or 900 153 862, for "Séneca" also citaprevia.alicante.es; one appointment covers several people at the same address;
- according to the council, all padrón procedures are free of charge;
- non-EU foreigners without permanent residence must renew their registration every two years (art. 16.1 of Ley 7/1985, as stated on the form itself).
What the form asks
- home address: street type and name, number, letter, km, block (bloque), entrance (portal), staircase, floor, door;
- voluntary: authorisation for the adults on the sheet to report changes and obtain certificates for everyone, phone and e-mail;
- for each of up to four people: given name, first and second surname, sex, date of birth, province and town or country of birth, nationality, type and number of ID (DNI, passport, NIE), passport for EU citizens, education code;
- reason: change of residence, omission, birth, change of address within the city or correction of data; when moving in — where from (town or consulate, province or country);
- "Autorización de persona empadronada" — consent of an adult already registered at the home; the number of people on the sheet and the line "En Alicante, a … de … de …".
What to bring
- the completed form signed by all adults;
- a valid ID for each person: DNI; EU citizens — the registration certificate with the NIE plus their passport or national ID; other foreigners — a TIE card or passport;
- proof of the home: ownership document, or a valid rental contract with the latest rent receipt; when joining people already registered — their consent, copies of their IDs and the housing document in their name;
- for children — libro de familia or birth certificate and the parents' consent;

How to fill it in with FoxDocs
