What this document is

EX-17 (“Solicitud de Tarjeta de Identidad de Extranjero (TIE)”) is the official Spanish form to apply for the TIE, the foreigner identity card. The card documents the stay or residence permit you have been granted and shows your NIE, photo and fingerprint. The form is in Spanish and has three pages: two to fill in and one with instructions.

The card itself does not give you the right to live in Spain: it is issued on the basis of a permit or visa you already hold. EX forms must be submitted in the official model: FoxDocs only types your values into the fields of the official PDF and does not change its layout.

Who files it and where

  • a foreigner granted a residence permit or a visa for more than six months — for the first card (tarjeta inicial);
  • a card holder whose permit has been renewed — for a new card (renovación);
  • after loss, theft, damage of the card or a change of details — for a duplicate (duplicado);
  • in person only, at a National Police unit (Comisaría or Oficina de Extranjeros) with a cita previa for “toma de huellas” (fingerprinting): the form is not accepted by post or online;
  • the first card must be requested in person within one month of entering Spain or of the permit being granted or taking effect (for work permits, depending on Social Security registration) — art. 209 RD 1155/2024;
  • the fee is paid with form 790 code 012; per the National Police fee table as of 2026-10 a first temporary residence card is €16.08, a renewal €19.30 and long-term residence €21.87; check the amount on sede.policia.gob.es before paying.

What the form asks for

  • personal data: passport, NIE, surnames, name, sex (H/M/X), date of birth (dd/mm/aaaa), place and country of birth, nationality, civil status (S/C/V/D/Sp), father’s and mother’s names;
  • address in Spain: street, number, floor/door, town, postcode, province; mobile phone and e-mail;
  • legal representative, if any: name, DNI/NIE/passport and title of representation;
  • representative for submitting the application (section 2), address for notifications (section 3) and consent to electronic notifications via DEHú;
  • section 4: type of application — first card, renewal or duplicate;
  • place, date, signature and, in the footer, the office the application is addressed to (DIR3 code and province).

Documents usually brought along

  • the EX-17 form, original and a copy, filled in in capital letters;
  • passport, original and a copy;
  • a photograph meeting Spanish DNI requirements (recent, in colour); check with your police unit how many and in what format;
  • the receipt for the 790-012 fee;
  • the resolution granting the permit or your visa; some police units may ask for proof of address (empadronamiento) — check in advance;
  • for a duplicate — the damaged card or, after theft or loss, a document about it (for example a police report, denuncia); check the list for your case with your police unit.

What is the EX-17 form for?

To get the TIE foreigner identity card — for the first time, after your permit is renewed, or as a duplicate for a lost or damaged card.

What is the difference between the TIE and the NIE?

The NIE is your foreigner identification number. The TIE is the plastic card showing that number, your photo and fingerprint, and it proves your stay or residence permit.

Can I submit EX-17 by post or online?

No. The form itself states it can only be submitted in person at a National Police unit, usually with a cita previa (appointment).

When do I have to apply for the first card?

Under art. 209 RD 1155/2024 — in person, within one month of entering Spain with your visa or of the permit being granted or taking effect (for work, depending on Social Security registration).

How much does it cost and how do I pay?
