What this document is
EX-15 (“Solicitud de Número de Identidad de Extranjero (NIE) y Certificados”) is the official Spanish form to request an NIE (Número de Identidad de Extranjero, the foreigner identification number) or a certificate of resident or non-resident status (art. 205 RD 1155/2024). The form is in Spanish and has three pages: two to fill in and one with instructions.
The NIE is an identification number, not a residence or work permit. EX forms must be submitted in the official model: FoxDocs only types your values into the fields of the official PDF and does not change its layout.
Who files it and where
- a foreigner who needs an NIE for economic, professional or social interests in Spain (for example, buying property or signing a contract), or a certificate of resident or non-resident status;
- in Spain: at the Oficina de Extranjería (foreigners’ office) or the Comisaría de Policía (National Police station), usually with an appointment (cita previa);
- abroad: at the Spanish consular office (Oficina Consular);
- the fee is paid with form 790 code 012 (tasa 790-012): per the National Police fee table as of 2026-10 it is €9.84 for an NIE and €7.31 for a certificate; check the amount on sede.policia.gob.es before paying;
- under art. 205 RD 1155/2024 the NIE is requested in person; section 2 (representative for submission) is filled in only if someone other than the applicant submits — check with the office in advance whether this is accepted in your case.
What the form asks for
- personal data: passport, NIE (if you already have one), surnames, name, sex (H/M/X), date of birth (dd/mm/aaaa), place and country of birth, nationality, civil status (S/C/V/D/Sp), father’s and mother’s names;
- address in Spain: street, number, floor/door, town, postcode, province; mobile phone and e-mail;
- legal representative, if any: name, DNI/NIE/passport and title of representation;
- representative for submitting the application (section 2), address for notifications (section 3) and consent to electronic notifications via DEHú;
- section 4: type of document (NIE or resident/non-resident certificate), reasons (economic, professional, social, other — specify), place of submission and situation in Spain (estancia or residencia);
- place, date, signature and, in the footer, the office the application is addressed to (DIR3 code and province).
Documents usually brought along
- the EX-15 form, original and a copy, filled in in capital letters;
- passport, original and a copy;
- documents proving the reason for the NIE (economic, professional or social);
- the receipt for the 790-012 fee;
- if the office accepts submission by a representative: a document proving their authority;
- the exact list depends on the office — check in advance.
How to fill it in with FoxDocs
