851F2 (“Solicitud de modificación de los datos recogidos en el Registro de Uniones de Hecho de la Comunidad de Madrid”) is the official form a member of a registered couple uses to ask the Madrid region’s Register of Partnerships to update the record. Two pages to fill in and two pages on data protection, in Spanish.
The form offers two kinds of change: address (domicilio) and identity document details. Madrid Law 11/2001 (art. 1.2) applies to couples in which at least one member is registered (empadronado) and lives in the region, so a move is worth recording.

What the form asks
- type of change: address or identity document details;
- applicant: NIF/NIE, passport, two surnames, given name, e-mail, fax, phones;
- representative (if any): NIF/NIE, surnames, given name, e-mail, fax, phones;
- notifications: electronic (only for users registered in the Comunidad de Madrid notification system) or registered mail — with a postal address;
- documents attached, refusal of online data checks, place and date of signature.
What to bring

- proof of representation — if a representative files;
- form 030 receipt — if a fee applies to the change;
- the register checks the NIF/NIE and municipal registration online; if you refuse, attach those documents. Confirm the exact list for your case with the register.
