What this document is
The Hoja de inscripción en el Padrón Municipal de Habitantes is the form of Cartagena city council (Ayuntamiento de Cartagena, Estadística service) for municipal registration (empadronamiento) and for changing your registered data: moving to Cartagena from another town or from abroad, moving within the municipality, registering a newborn or someone never registered before (omisión).

Who files it and where
- everyone who habitually lives in the municipality of Cartagena (if you live in several, the one where you spend most of the year), foreigners included;
- in person, by appointment (cita previa) at the Registro General (C/ San Miguel, 8), OMITAS or the Registro auxiliar de Estadística; appointments on 968 128 818 and 968 128 848;
- online, in the council's Sede Electrónica: with a digital certificate the sheet is attached to an instancia general; it can also be sent with an e-mail login, but then you must come in by appointment within 10 working days to sign;
- every adult listed signs the sheet by hand (legal representatives sign for children);
- the council decides within three months at most; if it does not reply, the application is deemed granted (silencio estimatorio);
- non-EU foreigners without permanent residence must renew their registration every two years.
What the form asks
- home address: street type and name, number, km, block, entrance, staircase, floor, door, building or urbanización, the 20-character cadastral reference, postcode and optionally a phone;
- for each resident: given name and two surnames, type and number of ID document, sex, nationality, country, province and town of birth, date of birth;
- type of registration: moving from another town in Spain or from abroad (where from), moving within Cartagena, birth or omisión;
- highest completed education from the list on the form and, optionally, the title of the qualification;
- date, number of people on the sheet and, if someone is already registered at the home, the name and ID number of the person who authorises the registration.
What to bring
- the sheet signed by every adult being registered;
- each person's ID: Spanish citizens — DNI; EU citizens — registration certificate with NIE plus passport or ID card; other foreigners — TIE card with NIE or passport; documents must be valid;
- proof of the home: owners — title deed showing the 20-character cadastral reference; tenants — current lease and the last rent receipt (if the cadastral reference is not in them, a letter from the owner stating it);
- if someone is already registered at the home — their signature in the «Autorización de persona empadronada» section, a copy of their ID and proof of the home in their name;
- for children — libro de familia or birth certificate; if only one parent registers the child — the other parent's consent or the council's declaration form;
- the list may change — check it on the procedure page before your visit.
