What this document is

The “Solicitud de rectificación de errores” is an application to the Encargado del Registro Civil: an error has been found in a birth, marriage or death record and the applicant asks for the record to be brought into line with reality.

A common case for foreigners is a given name or surname recorded with a transliteration error, swapped letters, or a wrong date or place of birth or parents’ names. The form is one page, in Spanish, issued by the Comunidad de Madrid.

Who applies and where

  • an adult applicant (as printed on the form) — usually the registered person; check with the Registro Civil who else may apply in your case;
  • the form is addressed to the Encargado del Registro Civil and is submitted to the Registro Civil;
  • one application — one error in one record: the form has one line for the wrong value and one for the correct one;
  • whether an appointment (cita previa) is needed and which office to go to — check on the Comunidad de Madrid website or at the Registro Civil.

What the form asks

  • applicant: name and surnames, DNI (foreigners — NIE or passport), address, phone and e-mail;
  • type of record — birth, marriage or death, whose record it is, the Registro Civil, volume, page and section from the certificate;
  • what is wrong — a tick in a table of 22 items (name, surnames, sex, date and time of birth, place, parents’ names, nationality and other);
  • how it is recorded now and how it should read;
  • which documents prove the correct value;
  • consent to SMS or e-mail messages, place and date; signature by hand.

What to bring

  • the certificado literal (full certificate) of the record with the error — the form lists it explicitly;
  • documents showing the correct value: passport, DNI or TIE, birth or marriage certificates;
  • foreign documents are generally legalised or apostilled and translated into Spanish; check the exact requirements with the Registro Civil;
  • the applicant’s identity document.

Errors in a birth, marriage or death record: given name, surnames, sex, day, month, year and time of birth, place of birth, names of parents and grandparents, dates and places of marriage and death, marital status, nationality and other items.

My surname has a transliteration error — is this the right form?

Yes, it is a common case. Tick “Primer apellido” or “Segundo apellido”, write the surname as recorded and as in your passport, and attach the passport.

Can I list several errors in one application?

The form has one line for the wrong value and one for the correct one, so FoxDocs prepares one error per application. Ask the Registro Civil about any other errors.

What is a certificado literal and why is it needed?

It is the full copy of the registry entry. The form lists it among the attachments: it shows the error and the record details — volume, page and section.

How much does the correction cost?
