What this document is

TA.4 («Solicitud de rectificación de datos de vida laboral») is the official form of Spain’s General Treasury of Social Security (TGSS). It is used to ask for corrections to your work history report (informe de vida laboral): wrong start or end dates, missing periods of work, or a history split across several Social Security numbers.

Your vida laboral determines pensions, unemployment benefits and other payments, so a missing period is worth fixing early.

Who files it and where

  • anyone insured with Spanish Social Security whose vida laboral contains an error or lacks a period of work;
  • anyone who has had more than one Social Security number and needs the record merged;
  • the form is submitted to an Administración de la Seguridad Social (TGSS) or through a public registry; it is signed by hand;
  • if you worked in several provinces you do not need to apply in each one: according to the form, the office where you file will gather your whole work history.

What the form asks

  • 1 — the insured person: surnames and name, Social Security number, date of birth, identity document, address, phone and e-mail;
  • 2 — any other Social Security numbers you have had;
  • 3 — the year your working life began and the insurance schemes you belonged to (general scheme, self-employed, household employees, seafarers, agriculture and historical schemes before 1970), with periods;
  • 4 — the companies you worked for: name, activity, province, start and end dates. This section is only filled in to correct data or add missing periods.

Dates may be approximate or incomplete — the form’s footnotes say so explicitly.

What to bring

  • your identity document (DNI, foreigner card NIE/TIE or passport);
  • the vida laboral report showing the error;
  • anything proving the period of work: employment contract, payslips (nóminas), an employer’s certificate.

When do I need TA.4?

When your vida laboral shows wrong dates, misses a period of work, or your history is split between several Social Security numbers.

Do I need exact dates?

No. The form accepts approximate or incomplete dates, for example only the month and year.

I worked in several provinces. Do I apply in each?

No. According to the form, the Social Security office where you file will gather your whole work history.

Which of the 16 schemes in section 3 do I tick?

Only those you were actually insured under. For most foreigners that is the general scheme (employees), self-employed (autónomos) or household employees.

What goes into section 4?

The companies whose period is missing or wrong: name, activity, province, start and end dates.

Is the form available in other languages?

Yes: the form says that regions with a co-official language have a version in that language. FoxDocs fills in the Castilian version.
