What this document is
“Solicitud de prestación económica por cuidado de menores afectados por cáncer u otra enfermedad grave” (form 8-015, commonly CUME) is the official form of Spain’s National Social Security Institute (INSS). A parent, adoptive parent, guardian or foster carer who has reduced working hours by at least 50 % uses it to claim a benefit for caring for a child with cancer or another serious illness requiring long hospital stays and treatment.
The benefit is governed by Real Decreto 1148/2011. The current edition, 8-015 cas V.13 (20250423), is downloaded from the Seguridad Social website.
Who files and where
- an employee or self-employed worker insured with Seguridad Social, including foreign nationals with an NIE;
- as a rule both parents must work; the form’s footnotes give exceptions — divorce, separation, annulment, end of partnership and accredited gender violence;
- online — Sede Electrónica of Seguridad Social (digital certificate or Cl@ve), or run.gob.es/tramites without electronic ID;
- by post — a signed copy of the form;
- in person — at a Social Security centre (CAISS) by appointment: 901 10 65 70 / 91 541 25 30 or run.gob.es/trdcita.
What the form asks
- the applicant’s details, address, phones, e-mail and relationship to the child;
- whether it is a single-parent family and whether there is accredited gender violence;
- the other parent: ID, Social Security number, whether they work and under which scheme;
- up to two children: surnames and name, ID (mandatory from 14), date of birth, disability of 65 % or more;
- the child’s hospital admission date, the start date and percentage of reduced hours;
- co-official language, a different address for letters, remarks, IBAN in the applicant’s name, consents, place and date of signing.
What to bring
The main items from the document list in the form itself:
- ID of the applicant, the other parent and the child aged 14 or over: DNI, passport, NIE or TIE;
- a public health service doctor’s statement on the illness, hospital admission and expected treatment;
- the employer’s certificate of reduced hours (date and percentage) and contribution base; self-employed — a declaration on reduced activity;
- the Libro de familia or a Civil Registry certificate; for adoption or guardianship — the court or administrative decision;
- if relevant — proof of the child’s 65 % disability, the divorce or partnership-end decision, proof of gender violence, proof of representation.
If documents are missing, INSS will ask for them; you have 10 days from notification to provide them.
