What this document is
«Hoja de inscripción o modificación» is Annex III to the INE instructions for town halls on keeping the municipal register (Resolución de 17 de febrero de 2020, consolidated text of 2023). It is the model padrón sheet: you use it to register as a resident, report a move within the town or correct your personal data.
Under Law 7/1985 everyone living in Spain must be registered on the padrón of the municipality where they usually live. Registration proves your address and is needed for many other procedures — from seeing a doctor to immigration paperwork.
Who files it and where
- residents of the municipality — Spanish citizens and foreigners — when registering for the first time, moving from another town or country, moving within the town, after a birth or when personal data change;
- one sheet per home and up to four people; every adult listed on the sheet signs it;
- it is handed in at the town hall (ayuntamiento) where you live — in person or as the town hall indicates;
- this is the INE model for town halls without their own form. Large cities have their own sheets (Madrid, Seville, Málaga, Alicante and others are in the catalogue separately). Check with your town hall that it accepts this model.
What the form asks for
- the home address: street type and name, number, letter, km, block, entrance, staircase, floor, door;
- for each resident: first name and both surnames, sex, date of birth, province and town or country of birth, nationality;
- ID: DNI, passport or NIE and its number, the education level code from the list on the back;
- the reason: move from elsewhere, «omisión», birth, move within the town or change of data; where you came from;
- optionally — permission for the adults on the sheet to report changes and obtain certificates, a phone number;
- consent of a person already registered there, if anyone is; the number of people on the sheet, place and date.
What to bring
- the sheet, filled in and signed by every adult;
- ID: Spaniards — DNI; EU, EEA and Swiss citizens — the registration certificate with the NIE plus a passport or national ID; others — a TIE card with the NIE or a passport;
- for children under 14 without ID — the libro de familia or birth certificate;
- proof of the home: title deed or a current rental contract with the latest rent receipt; with a resident’s consent — that person’s proof of the home and a copy of their ID;
- check the exact list, appointment and filing method with your town hall.
How to fill it in with FoxDocs

- Check the values: they are printed in capital Latin letters, the date of birth as dd/mm/yyyy, countries in Spanish.
- Download the PDF, print it, have every adult sign it and take it to the town hall.
