What this document is
“Requerimento para Transferência de Prontuário” is the official form of the Driver Licensing Directorate (Diretoria de Habilitação) of DETRAN-RJ, the traffic department of the State of Rio de Janeiro.
DETRAN-RJ says to use it to request the transfer of a prontuário — the driver record kept for a licence. The form names the state the record comes from (UF de origem). It is in Portuguese and has one page; FoxDocs writes your data into the official PDF without changing its layout.
Who files it and where
- a driver whose record is held in another state; the request can be made by the driver, a relative in the direct line over 18 (ascendente/descendente), a spouse (cônjuge) or another representative — marked in the “Quem requer o serviço” section;
- the applicant signs by hand with the signature certified (reconhecer firma por semelhança) or uses a digital signature (Certificação digital);
- the form is filed with DETRAN-RJ together with the documents listed on it;
- where it is accepted and what the fee is — check on detran.rj.gov.br.
What the form asks for
- full name and social name (nome social), if any; identity document and issuing body (órgão expedidor); CPF;
- the DUDA number (Nº do DUDA) — the fee payment document;
- date of birth, marital status (estado civil), place of birth (naturalidade), nationality;
- mother's and father's names (filiação);
- address (street, number, complemento, CEP, state, city, neighbourhood), phone numbers and e-mail;
- if you live in someone else's home — the owner's section (Termo de Concordância): name, nationality, place of birth, identity document, CPF, CNH, phones, e-mail, a statement that you live at the address, the owner's date and signature;
- record data (Dados do Prontuário): registration number (Registro Nacional), category (Cat.) and the state where the record is now (UF origem);
- who requests the service: the applicant, an ascendant/descendant over 18, a spouse or another person (specified); date and signature.
What to bring
- the “Documentos necessários” list on the form: DUDA (code 206-2), identity document, CPF, proof of address (comprovante de residência), CNH;
- if a representative files — their identity document and, depending on the case: ASD / Certificado Analítico (for a despachante), a private power of attorney (procuração por instrumento particular) with the OAB card when a lawyer acts, or a notarial power of attorney (procuração por instrumento público) for a third party;
- check the fee amount and the exact list on the DETRAN-RJ website.
