What this document is
“Declaração de Residência” (DETRAN – No. 0034) is an official form of DETRAN-RJ, the traffic department of the state of Rio de Janeiro. In it you declare yourself the address where you live.
DETRAN-RJ says to use it only when a person has no documentary proof of address at all (comprovante de residência) — such as an electricity, water or phone bill or a letter from a bank. The form is linked in the agency’s forms sections for driving licences (habilitação), vehicles and traffic offences. Who files it and where
- the applicant — the person whose address must be proven; the signature “Assinatura do requerente” is handwritten;
- the declaration is filed with DETRAN-RJ together with the service that requires proof of address;

- phone, mobile and e-mail;
- home address: street, number, complemento, CEP, state (UF), city, neighbourhood (bairro);
- the date in the line “Rio de Janeiro … de … de …”; the signature is handwritten.
