What this document is
“Formulário padrão de autorização de viagem internacional para crianças ou adolescentes” is the standard form of Brazil's National Council of Justice (Conselho Nacional de Justiça, CNJ) under Resolution 131/2011, version of 2024-04-05. Parents or a guardian use it to allow a minor to travel abroad with the companion named on the form.
According to the CNJ, the authorization is required when a Brazilian child or adolescent travels abroad without one or both parents. This form is for travel accompanied by a named adult (the “na companhia de” field is mandatory). The form is in Portuguese and one page long; a printed note says that, unless expressly stated, it does not authorize settling permanently abroad.
Who files it and where
- a child travelling with one parent needs written consent from the other (the travelling parent is named as the companion); a child travelling with other adults needs consent from both parents or legal guardians (CNJ leaflet);
- signatures are certified at a notary office (firma reconhecida em cartório por autenticidade ou semelhança); the CNJ says signing before the notary is not required — a signature already on file at the office is enough — and no photo of the child is needed;
- one authorization per child, in two copies: one is kept by the Federal Police (Polícia Federal) on departure;
- instead of paper, an electronic authorization (AEV) can be made by a notary on the e-Notariado platform (Provimento CNJ 103/2020);
- according to the PF, the authorization is checked only when leaving Brazil, not on the way back.
What the form asks
- the date until which it is valid (Válida até);
- one or two signers: name, ID card or passport number, issuing authority and date, address, city, UF, phone and capacity — father (PAI), mother (MÃE), guardian (TUTOR(A)) or custodian (GUARDIÃ(O));
- the child: name, date of birth, sex, place of birth (natural de), passport or ID number, issuing authority and date;
- the companion: name, passport or ID number, issuing authority and date, address, city, UF;
- place and date; signatures are handwritten.
What to bring
- two copies of the authorization with certified signatures;
- the child's valid passport;
- if applicable, the custody or guardianship order (termo de guarda ou tutela), as the CNJ leaflet reminds;
- ask the notary office about the cost of certifying signatures.
How to fill it in with FoxDocs
- Fill in the cards on this page — each field has a hint.
- The assistant asks step by step for the signers’, the child’s and the companion’s details and the validity date.
- Download the PDF, print two copies, sign by hand and have the signatures certified at a notary office.
