What this document is

“Formulário padrão de autorização de expedição de passaporte para menor” is a Brazilian Federal Police (Polícia Federal, PF) form under Article 27 of Decree No. 5.978/2006. The parents authorize a passport for their son or daughter under 18. The form is in Portuguese and one page long.

The form states that the authorization can be revoked only at the request of both parents or by court decision, which means immediate and irreversible cancellation of the child's passport. The PF stresses that the passport authorization and the international travel authorization are different documents: the first is needed at the PF appointment, the second on the day of departure.

Who files it and where

  • the form is filled in beforehand, but the parents sign it in front of the officer at the PF passport office;
  • the child must attend the appointment, even a baby; under 16, together with the parents or legal guardians;
  • if one parent will be absent, their signature must be certified in advance at a Brazilian notary office (por semelhança ou autenticidade); the PF also accepts other options, such as a specific power of attorney, an authorization made at a Brazilian consulate or a court order;
  • from 16 to 18 the minor may attend alone if both parents' signatures on the form are notarized;
  • if the birth certificate lists three or more parents, all of them must authorize; an emancipated minor needs no authorization.

What the form asks

  • for each of the two parents: full name, identity document number, issuing authority and date, CPF, phone with area code (DDD), address;
  • the child: full name, sex, date of birth, place of birth (city and UF);
  • place and date; two handwritten signatures.

What to bring

  • the form, signed in front of the officer or with the absent parent's signature notarized;
  • the child and the documents the PF requires for the passport — see the list on the PF website;
  • if a parent's name has changed since the child's birth, the original marriage or divorce certificate;
  • check the passport fee on the PF website.

How to fill it in with FoxDocs

  1. Fill in the cards on this page — each field has a hint.
  2. The assistant asks step by step for the parents’ and the child’s details.
  3. Download the PDF, print it and bring it to the PF appointment: sign it there, in front of the officer.

Frequently asked questions

Can I fill in PF passport authorization for a minor online?

Yes. FoxDocs fills in the original “PF passport authorization for a minor” form online: answer cards with hints (or add a photo of a document), check the values on the form and download the PDF. You submit it yourself: Signed and handed in at the Polícia Federal passport office appointment.

How long is the authorization valid?

One year from the date of signing or from the date the signature was certified at a notary office, according to the PF website.

Do we need a notary if both parents attend?

No. The form is filled in at home and signed in front of the PF officer at the appointment.

What if one parent is abroad?

The PF accepts, for example, the form with the absent parent's signature certified at a Brazilian notary office; a power of attorney or authorization made at a Brazilian consulate; or a foreign notarial document with an apostille and a sworn translation.

Can I sign with gov.br?

You can sign, but the PF states that an electronic signature does not replace certification: if the parent is absent, the signature still has to be certified. Electronic certification is currently done only through e-Notariado.

Must the child come along?

Yes. The PF requires the child at the appointment at any age, even a baby, because the officer checks the child's identity visually.

Is this the same as a travel authorization?

No. This authorization is for issuing the passport. Travelling without one or both parents requires a separate travel authorization, which can be printed in the passport if its type is expressly stated when applying.

How do I get the travel authorization printed in the passport?

The PF offers Tipo 1 (travel with either parent) and Tipo 2 (also unaccompanied). The type must be expressly named in the passport authorization; this form has no such line, and the matching version is on the PF website.

Can the authorization be revoked?

Only at the request of both parents or by court decision. The form warns that the child's passport is then cancelled immediately and irreversibly.
