What this document is
“Requerimento de Usuário dirigido à:” (VVD-Doc9002) is the official request form of DETRAN-GO, the traffic department of the state of Goiás. The agency’s website lists it as “Requerimento de Usuário (Requerimento Genérico)” and describes it as the form to request services from the Gerência de RENACH (the unit that keeps the national driver register).
On the form you tick the addressee: the President of DETRAN-GO (Presidência), one of the directorates — management, planning and finance; operations; technical and customer service — or another unit. One page in Portuguese; FoxDocs writes your data into the official PDF without changing its layout.
Who files it and where
- the applicant (interessado) — the person themself, who signs the form by hand;
- under item “b” of the form, the request is registered at the DETRAN-GO general registry (Protocolo Geral) or at a CIRETRAN, the local office;
