What this document is

The “Formulário de Identificação de Condutor” is the official form of DETRAN-PR, the traffic department of the state of Paraná, Brazil. The vehicle owner uses it to state who was driving when an offence recorded in an offence notice (auto de infração) took place.

Driver identification is provided for in art. 257 § 7 of the Brazilian Traffic Code (CTB) and art. 5 of Contran Resolution 918/2022. It does not apply to offences for which the owner is responsible (art. 257 § 2 CTB).

Who files it and where

  • the vehicle owner; both the owner and the driver sign, matching the signatures on the attached documents;
  • online on the DETRAN-PR website or in the PIÁ app, by registered mail to DETRAN-PR headquarters in Curitiba, or in person at a service unit;
  • only for offence notices under DETRAN-PR jurisdiction: the sender of the Notificação da Autuação shows where to file;
  • the deadline is stated on the notice; late identifications are not accepted. The service is free.

What the form asks

  • the issuing authority, the offence notice number and the number plate;
  • the owner’s name and document number, ticking CPF, CNH or RG;
  • the driver’s name, CNH number and CPF;
  • the deadline from the notice; signatures by hand.

What to bring

  • a legible copy of the owner’s identity document;
  • a legible copy of the driver’s valid CNH (or Permissão para Dirigir);
  • for a company-owned vehicle — documents proving the signer’s authority and the signer’s ID;
  • if a representative files — a power of attorney (notarial, or private with a certified signature).

Can I fill in DETRAN-PR — driver identification online?

Can I identify the driver after the deadline?

No. DETRAN-PR states that identifications filed after the legal deadline are not accepted. The deadline is on the notice and on the GIT Cidadão website.

Who must sign the form?

Both the vehicle owner and the driver, with signatures matching the attached documents.

Which owner document should I give?

One of the three ticked on the form: CPF, CNH or RG. Attach a copy of that same document.

Is a foreign driving licence accepted?

DETRAN-PR asks for a copy of the driver’s valid CNH or Permissão para Dirigir, and the form has a CNH number field. If the driver has no Brazilian licence, check with DETRAN-PR.

How much does it cost?

What if the car belongs to a company?

For company vehicles identifying the driver is mandatory; attach documents proving the signer’s authority and their ID. Failure to identify leads to a new fine (art. 257 § 8 CTB).
