What this document is
“Requerimento Geral” (DETRAN – No. 0049) is the official request form addressed to the President of DETRAN-RJ, the traffic department of the state of Rio de Janeiro.
DETRAN-RJ says to use it when a request does not fit any case that has its own form. The form is linked in every forms section of the agency: driving licences (habilitação), vehicles, civil identification and traffic offences. Who files it and where
- the applicant — the person themself or a representative holding a notarial power of attorney (procuração) with a certified signature; for a company — its representative with the company documents;
- the form is signed by hand and filed with DETRAN-RJ together with the documents listed on it;
- DETRAN-RJ may ask for the originals at any time to compare them with the copies;

What the form asks for
- name and social name (nome social), if you have one;
- identity document and the issuing body (órgão expedidor);
- Brazilian driving licence (CNH) number and expiry date, if you have one;
- nationality, place of birth (naturalidade), CPF, date of birth;
- address (street, number, complemento, CEP, state, city, neighbourhood), phones and e-mail;
- a five-line description of the service and the date; the signature is handwritten.
What to bring
- the general documents listed on the form: identity document, CPF, proof of address (comprovante de residência); for a company — CNPJ, company documents and its representative’s ID; with a representative — the power of attorney;
- documents for the specific service — the form lists: DUDA, CNH, CRLV, nota fiscal, guia de recolhimento de IPVA, GRM;
