What this document is
Form 364F1 (Anexo I) is the application to the Comunidad de Madrid for the título de familia numerosa, the official document proving large-family status. The same form is used for a first application, a renewal and a change when the family or its details change.
A large family is one or two parents with three or more children and, in some cases, with two (for example if one child has a disability or a parent has died). The special category covers five or more children, or four under the conditions of the law; the rest are general (arts. 2 and 4 of Ley 40/2003).
Benefits apply from the date the application is filed (art. 7 of Ley 40/2003).
Who files it and where
- either parent, a guardian, a foster parent or another family member with legal capacity;
- families living in the Comunidad de Madrid, including foreigners residing legally;
- EU/EEA nationals not living in Spain, if at least one parent works in the Comunidad de Madrid;
- electronically, or printed and signed at any public registry (art. 16.4 of Ley 39/2015), including by post.
What the form asks
- type of application: first issue, renewal (with the título number) or change (with the reason);
- both holders: NIF/NIE, surnames, given name, nationality, sex, marital status, disability, contact details;
- up to 10 children: given name, two surnames, date of birth, NIF/NIE, nationality, disability;
- family address and how you want to be notified (electronically or by registered post);
- list of documents attached, place and date of signing.
What to bring
- the family book (libro de familia) or marriage and birth certificates (foreigners — the equivalent from their country);
- foreigners — proof of legal residence (residence permit or EU registration certificate);
- a certificate of registration (volante de empadronamiento) no older than three months; children under six months need not appear on it;
- depending on your case: divorce and custody papers, disability certificate, proof of studies for children aged 21–25, income documents for the special category;
- if you do not object to the data checks — Anexo VI (form 364FA2): consent of the second holder and children over 18 to the check of their data;
- foreign documents with a sworn translation into Spanish and legalisation.
