What this document is

Form 364F2 (Anexo II of Decreto 141/2014) is the application to the Comunidad de Madrid for the individual card (tarjeta individual) of a large-family member. The card proves one person’s status, so the whole family’s título need not be carried around.

The card is issued on request to each family member included in the título who has a DNI or NIE, and it is valid for as long as the título (art. 14 of Decreto 141/2014). So a valid título comes first — it is obtained or renewed with form 364F1.

Who files it and where

  • the father, mother, a guardian, a foster parent or another family member with legal capacity — this person signs;
  • one application can request cards for several family members — the form has six rows;
  • electronically, or printed and signed at any public registry (art. 16.4 of Ley 39/2015).

What the form asks

  • applicant: NIF/NIE, surnames, given name, título number, e-mail;
  • how you want to be notified (electronically or by registered post) and the postal address;
  • who needs a card: given name, two surnames, date of birth and NIF/NIE of each;
  • the address the cards should be sent to;
  • a box to tick if you object to the authority checking the DNI/NIE; place and date of signing.

What to bring

  • the number of your valid título de familia numerosa;
  • the DNI or NIE of everyone who needs a card: the authority checks them itself, copies are needed only if you object to the check;
  • according to the form, consent of the other family members on Anexo VI for checking their data.

Frequently asked questions

Who gets an individual card?

Each family member included in the título de familia numerosa who has a DNI or NIE (art. 14 of Decreto 141/2014). That can be the parents as well as the children.

Do I need the título first?

Yes. The card is issued only for a valid título. If you do not have one yet or it has expired, file form 364F1 first to obtain or renew it.

How long is the card valid?

As long as the título it belongs to (art. 14 of Decreto 141/2014).

Can I request cards for several people at once?

Yes. Section 3 of the form has six rows — one for each person who needs a card.

Do I need to attach copies of the DNI/NIE?

Usually not: the Comunidad de Madrid checks them electronically. Copies are needed only if you tick that you object to that check (art. 28.2 of Ley 39/2015).

Is there a fee?

Under art. 14.8 of Decreto 141/2014, proof of payment of the applicable fee (tasa), or a request for exemption, is attached to the application. The form itself states no amount — check on the Comunidad de Madrid website whether a fee currently applies and how much it is.

How will I receive the decision?

Electronically only if you are registered in the Comunidad de Madrid electronic notification system; otherwise by registered post to the address you give.
