What this document is
Mod.04 “Solicitud de pruebas de aptitud” is the official form of Spain’s Directorate-General for Traffic (DGT) to apply for tests: for a driving licence or licence (permiso/licencia de conducción), for the dangerous goods authorisation (ADR) and for recovering a licence after losing all points (RPV).
Immigrants need it when their home licence cannot be exchanged (Spain has no agreement with that country) or when they have no licence: the Spanish licence is then obtained by passing the DGT theory and practical tests.
Who files it and where
- the candidate or a person they authorise (power of attorney in the DGT Registro de apoderamientos or a signed Mod.24 form);
- in person at any Jefatura or Oficina de Tráfico by appointment (cita previa); online through the DGT Registro Electrónico with an electronic DNI, digital certificate or Cl@ve;
- driving school students usually file through the school — the form then shows its number, section and check digit;
- one application per procedure; minors attach the consent of their father, mother or guardian with their DNI and proof of the relationship.
What the form asks
- applicant: DNI/NIE, date of birth, country of birth, nationality, given name, two surnames, sex, phone, e-mail;
- address for notices: street type and name, number, block, entrance, staircase, floor, door, km, postcode, province, municipality, town;
- the medical centre that issued the certificate and the boxes “lenses”, “restrictive conditions”, “limited validity”;
- for a licence: class, fee number, manual or automatic, test centre, driving school, theory test language, headphones and extra time;
- for ADR: obtaining or extending, and the courses (basic, tanks, classes 1 and 7) with training centre and dates;
- for RPV: profile, training centre, course dates and code; place and date — the signature is handwritten.
What to bring
- the completed and signed Mod.04;
- the medical fitness certificate (informe de aptitud psicofísica) from a drivers’ medical centre — DGT states it is valid for 90 days;
- the fee receipt, or a bank card to pay at the office (cash is not accepted at the Jefatura);
- ID: EU citizens — national ID card, or passport with the registration certificate; non-EU citizens — a valid residence permit;
- a photo (talón-foto 32 × 26 mm) — only for LCM and LVA licences and only the first time.
Fee and time limits
- licence (moped, motorcycle, car, lorry, bus) — tasa 2.1, €94.05; A2 to A — tasa 2.3, €28.87; LCM and LVA licences — tasa 2.2, €44.58; licence recovery — tasa 2.3, €28.87 (DGT figures on the check date);
- one fee covers two attempts (three for licence recovery);
- a passed theory test is valid for two years; you may apply up to three months before the minimum age for the class;
- for ADR the form lists tasa 2.3 for obtaining and 4.5 for extending — check the amount on the DGT website.
