What this document is

C-176 (“Registro del documento S-1 para cobertura de asistencia sanitaria en España”) is the official form of Spain’s National Social Security Institute (INSS). It is used to register in Spain an S1 form — the certificate of entitlement to healthcare issued by an insurer in another country.

The S1 is a European document: the country where you are insured (usually the one paying your pension or where you work) covers the cost of your healthcare in your country of residence. Once the S1 is registered, you receive healthcare in Spain on the same terms as people insured here.

Who files it and where

  • the person named on the S1: a pensioner of another EU/EEA country, Switzerland or the UK living in Spain, a posted worker or anyone else insured abroad;
  • family members of the insured person, if the S1 also covers them — they then enter the details of the insured person they depend on;
  • the form is addressed to the INSS Provincial Directorate of your province; you hand it in at an INSS office (CAISS) together with the original S1, or through an official registry. Check the Social Security website for whether an appointment is needed.

What the form asks

  • the State that issued the S1 (Estado de aseguramiento);
  • applicant details: surnames, given name, DNI/NIE, date of birth, address in Spain with postcode, phone, e-mail;
  • insured person’s details — only if the applicant is a family member;
  • which documents are enclosed, two consents (data checks with other authorities, messages to phone and e-mail), place and date of signing, INSS province.

What to bring

  • the original S1 form;
  • your DNI or NIE document and passport;
  • EU, Iceland, Liechtenstein, Norway and Switzerland nationals — the EU citizen registration certificate; other nationals — the TIE card;
  • family members — proof of kinship with the insured person (marriage or birth certificate).

Frequently asked questions

Where do I get the S1 form?

The S1 is issued by the insurer or pension body of the country where you are insured. Form C-176 does not replace it: it only registers an S1 you already have in Spain.

Who is this form for?

People named on an S1 issued by another EU/EEA country, Switzerland or the UK — usually pensioners of those countries living in Spain, posted workers and their family members.

What goes in “Estado de aseguramiento”?

The country that issued your S1, in Spanish — for example REINO UNIDO, ALEMANIA, FRANCIA. The assistant writes it for you.

When do I fill in the insured person section?

Only if you are a family member of the insured person and your S1 was issued to you as a family member. If the S1 is in your own name, leave it empty.

What if I do not consent to data checks?

As the form states, you must then provide the documents requested yourself within 10 working days.

Do I need a second surname?

The form requires it from Spanish nationals. Foreigners fill it in only if their passport shows a second surname.

Do I have to sign the form?

Yes, the applicant signs the paper copy by hand. There is no signature field in the PDF.
