What this document is

Form 6-071 (“Solicitud de reconocimiento del derecho a la asistencia sanitaria”) is the official form of Spain’s National Social Security Institute (INSS). It is used to have the right to public health care recognised for people who do not get it automatically, and to add family members (beneficiarios) to an existing entitlement.

Employees, the self-employed, pensioners, recipients of periodic Social Security benefits and unemployed people who have used up their benefit get the right automatically and do not need this form.

Who files and where

  • case A — a resident of Spain who does not work, receive a pension or benefit, and has no cover by another route (MUFACE, MUGEJU, ISFAS, the right to “export” cover from the home country);
  • case B — an existing holder (titular) adding a spouse or registered partner, children, grandchildren, siblings under 26 (older with a disability of 65 % or more), and children in guardianship or foster care;
  • case C — a public body acting as guardian of a minor;
  • if your spouse or partner already holds the right, he or she files and adds you as a beneficiary;
  • filing: online at the Social Security e-office, or a signed copy by post or at an INSS centre (CAISS) by appointment.

What the form asks

  • section I — applicant: surnames, name, date of birth, sex, marital status, DNI/NIE/passport, Social Security number, nationality, address, phones, e-mail;
  • section II — whether you have cover by another route and whether your spouse or partner is already a holder;
  • section III — up to four beneficiaries: details, relationship, income, other cover, other parent’s name and ID for children;
  • section IV — minor under administrative guardianship; section V — statements; consent to messages, place and date of signing, INSS province.

What to take with you

  • identity documents of the applicant and beneficiaries (DNI from age 14; foreigners — TIE, passport or national ID);
  • foreigners — proof of legal residence in Spain;
  • citizens of the EU, EEA, Switzerland, the UK, Andorra, Chile, Morocco and Tunisia — a certificate from their home institution that the health-care right is not exported to Spain;
  • for beneficiaries — proof of relationship (libro de familia, marriage or birth certificate, registered-partnership certificate) and, for disability, the disability certificate;
  • proof of representation if a representative files.

Frequently asked questions

Who does not need this form?

Workers, pensioners, recipients of periodic Social Security benefits and unemployed people who have used up their benefit: the right is recognised automatically.

Can I apply if I am insured in my home country?

No, if you can export cover from your home country or are covered by MUFACE, MUGEJU or ISFAS: the form is for people without other cover. EU citizens and some others need a certificate that no such right exists.

Whom can I add as a beneficiary?

A spouse or registered partner, children, grandchildren, siblings under 26 (older with a disability of 65 % or more), and children in guardianship or foster care.

How long does a decision take?

According to the INSS notice on the form, 30 days from registration of the application or from submission of the documents. Without a reply by then the application is deemed refused and a prior claim (reclamación previa) can be filed.

What if documents are missing?

INSS sends a request; the documents must be provided within 10 days of the notification — online, by post or at a CAISS.

Does a child need a DNI?

For children under 14 the DNI can be reported later; from 14 it is mandatory.

Do I have to sign the form?

The paper copy is signed by the person filing it. When filing online, no signature on the form is needed.
