What this document is

C-097 is the form of Spain’s National Social Security Institute (INSS) for requesting a CPS (Certificado Provisional Sustitutorio), a temporary certificate that replaces the European Health Insurance Card (Tarjeta Sanitaria Europea, TSE).

The CPS gives the same cover as the card: medically necessary care during a temporary stay in the EU, Iceland, Liechtenstein, Norway, the United Kingdom and Switzerland, under the rules of the host country. It is issued when the card cannot be produced, when specific documents must be submitted, or when the trip is imminent and the card would not arrive in time.

Who applies and where

  • the person insured with Spanish Social Security (titular), a dependent family member (beneficiario), or a representative of the insured person;
  • the paper form is filed at any CAISS office by appointment — the CPS is handed over on the spot;
  • with a digital certificate or Cl@ve permanente you can get the CPS online in the Portal de Prestaciones and print it at home — no form needed;
  • the CPS is not valid if the purpose of the trip is medical treatment, or if you move your residence to another country.

What the form asks for

The validity period (from and to), the applicant’s details: two surnames, first name, DNI/NIE, date of birth, Social Security number, actual address and the date you have lived there since, phones, e-mail, postal address. If a family member or a representative applies — the insured person’s details. Then two consents, place and date of signing and the province of the INSS directorate.

What to bring

  • the applicant’s DNI, NIE/TIE or passport;
  • if a representative applies — proof of representation and the insured person’s ID;
  • any documents the CAISS asks for in your case (for example, proof of the right to healthcare abroad).

How is the CPS different from the European Health Insurance Card?

The cover is the same. The CPS is a paper certificate for the period of a specific trip, at most 90 days; it is issued immediately, online or at a CAISS.

How long is a CPS valid?

For the period shown on the document, at most 90 days from its start date. Make sure your return date falls within that period.

What is the fastest way to get a CPS?

Online in the Portal de Prestaciones with a digital certificate or Cl@ve — immediately. Without them, at a CAISS by appointment with this form; it is issued on the spot.

Can I get a CPS for my child or spouse?

Yes, if they are a dependent family member of the insured person. The section on the insured person (titular) is then filled in too.

Does the CPS cover a trip for medical treatment?

No. Planned treatment in another country needs a separate INSS form issued after a favourable report from the health service.

What if I refuse consent to the data check?

Then you must bring the documents INSS asks for within 10 working days, as the form states.

Will I pay anything to a doctor abroad?

Sometimes: a fixed amount or a share of the costs, like local insured people. According to Social Security, these amounts are not refunded.
