What this document is

C-096 is a form of Spain’s National Social Security Institute (INSS): a claim to refund necessary medical treatment (tratamiento necesario) that a person insured in Spain paid for themselves during a temporary stay in another country.

According to the Social Security website, you can claim if you needed medical care while in an EU country, Iceland, Liechtenstein, Norway, Switzerland or the United Kingdom and had to pay all or part of it — even with the European Health Insurance Card (TSE) or a CPS certificate. If you could not get the refund in the country of stay, you can claim it after returning to Spain.

Who files it and where

  • the person insured in Spain (titular); if a family member was treated, the “persona asistida” section is filled in too;
  • instead of the insured person, a legal representative or a former spouse entitled after judicial separation or divorce may apply;
  • the paper form with the original invoices is filed at a CAISS office by appointment;
  • you can also apply online through the service at prestaciones.seg-social.es, linked from the Seguridad Social page.

What the form asks

Insured person’s details: surnames, given name, date of birth, DNI/NIE/passport, Social Security number, phone, e-mail, address and purpose of the trip. If needed, the patient’s and the applicant’s details. Then: dates, town and country of treatment, amount paid and currency, which originals are attached, why you had to pay, IBAN for the refund, two consents, place and date of signing, INSS provincial directorate.

What to bring

  • original invoices: doctor, hospital or clinic, prescriptions priced by the pharmacy, other services;
  • proof of payment: a bank statement or an invoice stamped as paid;
  • DNI, or for foreigners a passport or national ID together with the TIE or the EU citizen registration certificate;
  • if someone other than the insured person applies — proof of representation or the divorce judgment (notarial agreement).

Frequently asked questions

Who is this form for?

People insured in Spain who paid for necessary treatment themselves — for themselves or a family member — during a temporary trip to the EU, EEA, Switzerland or the United Kingdom.

Will I get the full amount back?

Not necessarily. According to Social Security, costs are refunded under the rules and tariffs of the country of treatment; co-payments that local insured people also pay are not refunded.

Can I apply online?

Yes. The Seguridad Social page offers an online service at prestaciones.seg-social.es; the paper form is for applying in person.

Do I need the original invoices?

Yes. The form states that the original invoices proving payment must be submitted.

Does it cover planned treatment abroad?

No. For treatment you travelled for (form S2), INSS has a separate form — tratamiento programado.

What if I do not give consent to data checks?

Then you must bring the documents INSS asks for yourself within 10 working days, as the form says.

Is there a deadline?

The Social Security page does not state one. Check at a CAISS office or through the enquiry mailbox, topic “Asistencia Sanitaria Internacional”.
