What this document is
“Solicitud de tarjeta / alta / modificación de datos en BDU” is the official form of the Servicio Andaluz de Salud (SAS). It is used to request the Andalusian health card (tarjeta sanitaria), register in the SAS user database (BDU), change doctor or update your details. The form is in Spanish, edition of 2026-03-30: the same form in two copies plus data-protection pages.
According to SAS, the health card identifies you to the Andalusian health service, has no expiry date and exists in physical and virtual form. The virtual card is available only once a physical card has been issued at least once. A first card also means registration in the BDU and assignment of a primary-care centre and a family doctor or paediatrician.
Who applies and where
- Andalusian residents with public health cover recognised by the INSS (Instituto Nacional de la Seguridad Social), at any age — children should have their own card from birth;
- from age 14 a DNI is required, or an NIE for resident foreigners;
- people temporarily in Andalusia from other Spanish regions and foreigners — for temporary registration (alta temporal);
- the form is handed in at a primary-care centre (centro de atención primaria / centro de salud); according to SAS it may also be filed at any official registry under Law 39/2015;
- SAS offers replacement of a lost, stolen or damaged card online via ClicSalud+ or the App Salud Andalucía; the new card is posted to the address held in the BDU.
What the form asks
- section 1: what you request — a card (first time, not received, data error, loss, damage, virtual card actions), registration or change of details, a doctor during a stay elsewhere in Andalusia, temporary registration;
- section 2: name and surnames, nationality, sex, date of birth, country of birth, document type and number, NUHSA and NAF (if any), cover scheme (mutualidad), address, temporary stay details, phones and e-mail;
- section 3: requested doctor and centre (optional);
- section 4: legal representative — for children and people who cannot act alone;
- section 5: place, date and signatures.
What to bring
- the completed form;
- DNI or NIE of the insured holder — physical or digital;
- DNI of beneficiary family members over 14 and the libro de familia if a beneficiary is under 14;
- a certificate of municipal registration (certificado de empadronamiento) proving residence in Andalusia;
- for a newborn: the INSS must first recognise the baby as a beneficiary of the father or mother; then bring the form, the parent’s or guardian’s ID and the libro de familia to the centre.
