What this document is
C-147 (“Solicitud de opción de asistencia sanitaria”) is the official form of Spain’s National Social Security Institute (INSS). With it, a person whose right to public healthcare is already recognised as an independent holder (titular) chooses another status: becoming the beneficiary (beneficiario) of another Social Security holder or, for SOVI pensioners, moving to the civil servants’ schemes.
INSS only processes the application in the situations listed on the form; in any other case it is not dealt with.
Who files and where
- case A — a holder whose unemployment benefit or allowance has run out, who has a special agreement (convenio especial), is on leave of absence (excedencia), is covered as a resident of Spain (not working, not a pensioner and not receiving Social Security benefits) or whose entitlement is extended (prórroga), and who wants to become the beneficiary of another Social Security holder;
- case B — a SOVI pensioner who wants to become the beneficiary of a MUFACE, MUGEJU or ISFAS holder;
- case C — a SOVI pensioner who wants to become a holder as a recipient of a survivor’s pension (viudedad, orfandad, favor de familiares) under those schemes;
- for a minor or an adult with a disability, a representative files;
- to become the beneficiary of a MUFACE, ISFAS or MUGEJU member in other cases, you apply directly to the mutual society, not to INSS;
- filing: Social Security e-office (digital certificate or Cl@ve), run.gob.es, a signed copy by post or at an INSS centre (CAISS) by appointment.
What the form asks
- section I — current basis of entitlement: one of six situations;
- section II — applicant: surnames, first name, DNI/NIE/passport, date of birth, mobile, e-mail;
- section III — the person the option is made for (only a minor or an adult with a disability);
- section IV — the holder whose beneficiary you become (case A only);
- section V — statements, consent to messages, place and date of signing, INSS province.
What to bring
- proof of identity: DNI for Spanish citizens; for foreigners, TIE, passport or national identity card;
- proof of representation or of support measures for a person with a disability, if a representative files;
- in case A, proof of the relationship with the holder: libro de familia, marriage or birth certificate, registration certificate of the pareja de hecho;
- for a child under three months, proof of relationship is not required; for children, grandchildren or siblings over 26, also a disability certificate of at least 65%;
- if you were already a beneficiary of the same holder, no proof of relationship is needed.
