“Declaração, sob as penas da lei, de que o familiar chamante reside no Brasil” is a model published by the Brazilian Federal Police (Polícia Federal, PF) under “Declarações e Formulários” (file dated 25/01/2021). It is used in the residence procedure for family reunion (reunião familiar).
The chamado is the person applying for residence; the chamante is the family member they are joining, who lives in Brazil. Under penalty of law, the declarant states that the chamante lives in Brazil at the given address. The form carries a printed warning about Article 299 of the Penal Code (false statements in a document).
Who files it and where
- the declarant signs it — usually the family member applying for family reunion residence;
- the signed declaration is attached to the request filed with the PF together with the other documents;
- under Decree 9.199/2017 (Art. 153, § 2), family reunion residence is not granted if the chamante themselves holds residence by family reunion;

What the form asks
- full name, nationality, marital status, date and city of birth of the declarant;
- names of the declarant’s father and mother (filiação);
- the chamante’s full name, date of birth and identity document number;
- the chamante’s address in Brazil with CEP and phone number with DDD area code;
- city, UF and date of signing; the signature is handwritten.
