What this document is
“Declaração … nos cinco anos anteriores à data da solicitação” is a template of Brazil’s Federal Police (PF). The declarant states, under penalty of law, that they had no police inquiry, criminal case or conviction — in Brazil or abroad — in the five years before the date of the request.
The form is in Portuguese, one page; at the bottom it warns about Article 299 of the Brazilian Penal Code (false statements in a document).
Who files it and where
- the foreign national signs the declaration personally; it is attached to the migration request to the PF with the other documents;
- the wording follows the general requirement of Decreto nº 9.199/2017 (art. 129, VI) for residence applications: a declaration of no criminal record in any country in the five years before the request;

What the form asks for
- full name, nationality and marital status;
- date and city of birth;
- father’s and mother’s names (filiação);
- city, state (UF) and date of signing; the signature is handwritten.
