What this document is

T.3 ("Solicitud de documento o información existente en las bases de datos de la TGSS sobre personas físicas") is the official form of Spain's General Treasury of Social Security (TGSS). It is used to request information that the TGSS holds about a person.

One form can tick several items: a duplicate of the document showing your Social Security number, a report certifying you are not affiliated, a work history report (informe de vida laboral) — complete or partial, and information on contribution bases and on payment of contributions.

Who files it and where

  • the insured person (afiliado/a) — a foreign national or a Spanish citizen;
  • if another person or a company (for example a gestoría) collects the document, page 2 — the authorisation — is filled in and signed by both sides;
  • the form is handed in at a TGSS office (Administración de la Seguridad Social) or through a public registry — page 1 has boxes for the "Registro de presentación" and "Registro de entrada" stamps;
  • some of these reports, including vida laboral, are also issued electronically through the TGSS Sede Electrónica.

What the form asks

  • details of the insured person: surnames, name, Social Security number, date of birth, type and number of ID document, address, phone, e-mail;
  • other Social Security numbers, if you have had more than one;
  • what you request: duplicate document, non-affiliation report, vida laboral (complete or partial — for Clases Pasivas or for periods and schemes), contribution information;
  • why you need the information, place and date of signing;
  • on page 2 — details of whoever collects the document for you.

What to bring

  • the insured person's identity document: DNI, foreigner card or passport (original or copy, as the form states);
  • if someone else collects it — a copy of the insured person's valid ID and the original ID of the authorised person or of the company representative.

Frequently asked questions

What is vida laboral?

It is the TGSS report of your working life: under which schemes and for which periods you were registered with Social Security. Banks, employers and authorities often ask for it.

What is the difference between a complete and a partial report?

A complete report shows your whole history. A partial one is requested for Clases Pasivas del Estado or for chosen periods and schemes, which you then mark on the form.

Can I get a report without a Social Security number?

The form has a box for the number, but if you were never registered you have none — in that case the non-affiliation report is the relevant item.

Can someone else collect the report?

Yes. Page 2 of the form is completed for that: details of the authorised person or company and signatures. A copy of the insured person's ID and the original ID of the collector are needed.

How much does it cost?

The TGSS page for form T.3 does not mention a fee. Check with the TGSS office before filing.

Do I have to write in capitals?

Yes, the TGSS form page says fields must be filled in capital letters. FoxDocs does this for you.

Who needs information on payment of contributions?

Only people responsible for paying their own contributions: self-employed workers, sales representatives, agricultural and sea workers, discontinuous household employees and special-agreement holders.
