What this document is

T.5 («Autorización de entrega de información a persona distinta a su titular») is the official form of the Spanish General Treasury of Social Security (TGSS). With it the data holder authorises someone else to collect documents and reports about them at a TGSS office.

The form lists what may be released: the Social Security number document, the contribution bases report, the contribution payment status report, the employment history report (vida laboral) of a worker or a company, and other information.

Who files it and where

  • the data holder — an individual (foreigner or Spanish citizen) or an organisation acting through its representative;
  • the authorised person — another individual or an organisation (e.g. a gestoría) with its representative;
  • the authorised person shows the form at the TGSS office where the documents are collected; page 1 has boxes for the «Registro de presentación» and «Registro de entrada» stamps;
  • form T.3 (request for a report on an individual) has its own authorisation on the back; T.5 is a stand-alone authorisation in which the holder can also be an organisation.

What the form asks

  • holder: surnames, given name, document type and number, Social Security number, address, phone, e-mail — or, for an organisation, its name, CCC, NIF, address and representative;
  • authorised person: the same details for the individual or organisation;
  • which documents may be released;
  • place, date and signatures of the holder and the authorised person and, for an organisation, its stamp and the representative's signature.

What to bring

  • the original identity document of the authorised person;
  • the original or a copy of the holder's identity document — as the footnote of the form says.

What is form T.5 for?

It lets another person or organisation collect your TGSS documents (vida laboral, NSS document, contribution reports) at an office.

Who can be the authorised person?

Any individual or organisation, such as a gestoría. For an organisation you also give the representative who will come.

Which documents can be released with T.5?

The Social Security number document, contribution bases and payment reports, the vida laboral of a worker or company and other information you write in.

What should be brought to the office?

The authorised person shows their original ID and the original or a copy of the holder's ID, as stated on the form.

How is T.5 different from page 2 of T.3?

The back of T.3 is an authorisation inside the request for a report on an individual. T.5 is a separate form: the holder can also be an organisation, and it has its own list of documents, including vida laboral de empresa.

Must it be filled in capitals?

Yes, the TGSS form page says the fields must be filled in capital letters. FoxDocs does this automatically.
