What this document is
INC-17 (“Solicitud de incapacidad permanente y lesiones permanentes no incapacitantes”) is the official form of Spain’s National Social Security Institute (INSS). A person insured with Spanish Social Security uses it to ask for recognition of permanent incapacity for work and the related benefit, as well as compensation for permanent injuries that do not cause incapacity.
The degree of incapacity is assessed by the INSS medical board — the form does not state it. In the form you give the cause (common illness, occupational disease, accident at work or outside work), your last occupation, your work plans after recognition, children, income and bank account.
Who files and where
- an insured employee or self-employed person (autónomo) whose illness or injury has reduced their capacity to work in the long term; a supporting person or a representative may file for them;
- online — at the Social Security e-office (sede.seg-social.gob.es) with a digital certificate or Cl@ve; without them — via run.gob.es/tramites;
- on paper — a signed copy by post or at a CAISS centre by appointment (the form lists 901 10 65 70 and 91 541 25 30);
- if you have worked abroad (EU, EEA, Switzerland or a country with a bilateral agreement), the form warns you to check first whether an EU-regulation or agreement form is required.
What the form asks
- personal details, NIE/DNI or passport, Social Security number, address; if needed, the supporting person and the representative;
- a mobile number and/or e-mail for notifications — mandatory, since the INSS notifies only electronically in this procedure;
- last occupation and duties, other occupations, whether a self-employed applicant had employees;
- the cause of incapacity and your plans: will you keep working, hold a farm or business, do you claim the 20 % increase;
- children — for the childcare benefit, the gender-gap supplement and days credited for childbirth;
- annual income of the applicant and spouse, spouse details, relatives living with you and tax data — for minimum top-ups and IRPF withholding;
- a bank account in the applicant’s name, consents, place and date of signing.
What to bring
- proof of identity: DNI for Spanish nationals; for foreigners, the TIE or a passport plus NIE;
- for an accident at work or occupational disease — the official report (parte) and the employer’s certificate of last year’s salary;
- for a supporting person or representative — proof of their authority;
- for minimum top-ups: foreigners — TIE or Central Register of Foreigners certificate; marriage and birth records, adoption decisions;
- foreign documents must be legalised for use in Spain; the INSS may ask for more in your case.
