What this document is

C-148 is a form of Spain’s National Social Security Institute (INSS) used to request free medicines (farmacia gratuita) for a minor with a recognised disability of 33 % or more.

The right is granted to the child; the application is made by the adult responsible for the child.

Who files it and where

  • a parent, guardian (tutor), foster carer (acogedor) or guardador of a child who does not yet have farmacia gratuita;
  • online — in the Social Security e-office (Sede Electrónica) with a digital certificate or Cl@ve permanente, or without them via run.gob.es; then the form is not needed;
  • on paper — a signed copy by post or at a CAISS office by appointment (phone 901 10 65 70 / 91 541 25 30).

What the form asks

Section I — the applicant: both surnames, first name, DNI/NIE/passport, date of birth, mobile and e-mail. Section II — the child: surnames, first name, ID document and date of birth. Section III — optional statements. Then consent to contact by phone and e-mail, place and date of signing and the INSS provincial office.

What to bring

  • the applicant’s ID: DNI, NIE or passport;
  • the child’s ID: NIE or passport; Spanish children over 14 — DNI;
  • the certificate recognising the child’s disability of 33 % or more;
  • a parent — the libro de familia or birth certificate; a guardian, foster carer or guardador — the document issued by the competent authority.

Frequently asked questions

Who can file C-148?

A parent, guardian, foster carer or guardador of a minor with a recognised disability of 33 % or more, if the right to free medicines has not yet been granted.

What if the child’s disability has not been recognised yet?

The disability certificate must be attached. Without it INSS will not grant the right, so obtain the certificate first.

How long does INSS take?

According to the form, up to 30 days from registration or from when all documents are provided. If there is no answer, the request is deemed refused and you may file a reclamación previa.

Is there a fee?

The form does not mention any fee. Check the details at a CAISS office or on the Social Security website.

Can I apply without the child’s DNI?

Yes. According to the form, the DNI of a child under 14 can be provided later; from 14 it is mandatory. Foreign children use their NIE or passport.

What if INSS asks for more documents?

They must be provided within 10 days of the notice — online, by post or at a CAISS office.

Does the form need a signature?

Yes, a paper application is signed by the applicant. An online application does not need the signed form.
