What TM.5 is

TM.5 (Thai: ตม.5, “การแจ้งเพิ่ม ลด หรือเปลี่ยนคนประจำพาหนะ”) is the Thai Immigration Bureau form by which the owner or master of a conveyance tells the immigration officer about changes to the crew compared with the crew list submitted on arrival.

The form is bilingual (Thai and English) and has two pages. Page 1 is a letter: the vessel or flight, the number of crew on the list, the date the list was submitted, what has changed and why. Page 2 is a table: family, first and middle name, age, nationality, former conveyance, position, number on the crew list, type of change (addition, reduction, change, remaining in Thailand), reason and remarks.

Who files it and when

It is signed by the owner or master (ผู้ควบคุมพาหนะ) of the vessel or aircraft; in practice the ship’s agent often prepares the paperwork.

The notification is needed when the crew changes after the crew list was submitted: someone signs off, joins, is replaced, or does not leave with the conveyance.

  • The notice printed on the form says that if a crew member does not leave with the same conveyance, the owner, master or agent must take him or her to the local immigration office forthwith.
  • The form does not state a filing deadline — check with the immigration checkpoint at the port or airport.

Where to file

With the immigration officer (พนักงานเจ้าหน้าที่ตรวจคนเข้าเมือง) — normally at the immigration checkpoint of the port or airport where the crew list was submitted. Check the local procedure there.

Documents usually attached

  • the TM.5 form, both pages, completed and signed;
  • a copy of the crew list submitted on arrival, if requested;
  • copies of the passports or seaman’s books of the crew concerned;
  • onward travel documents (e.g. tickets) for crew leaving the conveyance.

Requirements differ between checkpoints — confirm the list and any fees with your immigration checkpoint.
