What TM.4 is

TM.4 (Thai: ตม.4, บัญชีคนประจำพาหนะ) is the Thai Immigration Bureau crew list. It lists everyone working on board a conveyance — usually a ship or a yacht — when it arrives in or departs from Thailand.

The form is bilingual (Thai and English). At the top you tick ARRIVAL or DEPARTURE and give the name of the conveyance, its owner, the route, the Thai province of the port and the date. The table holds up to eight crew members: name, age, nationality, position on board, ID card / passport / seaman's book number and remarks.

Who files it

The crew list is signed by the master of the conveyance (captain) and by the owner or the agent acting for the owner. Crew members do not file it individually.

The lower boxes signed by the immigration officer and the immigration health officer are completed by the authorities, not by the applicant.

  • Passengers are not entered on the crew list — they are reported separately. Ask the checkpoint which forms it needs for your vessel.

At the immigration checkpoint of the port where the conveyance arrives or departs. The blank used here is published by the Samut Prakan immigration office: samutprakanimmigration.go.th. General information: immigration.go.th.

Procedures, number of copies and deadlines differ between checkpoints — check with the office before arrival or departure.

Documents usually attached

  • the TM.4 form signed by the master and by the owner or agent;
  • passports or seaman's books of all crew members listed;
  • the vessel's registration papers and clearance from the previous port, if the checkpoint asks for them;
  • other arrival or departure forms required for the vessel and its passengers.
