What this document is

EX-25 (“Solicitud de residencia temporal y desplazamiento temporal de menores extranjeros”) is the official Spanish form for foreign minors (Organic Law 4/2000 and Title IX of Royal Decree 1155/2024). It is used to apply for residence for a child born in Spain or joining their parents, for unaccompanied minors and former wards after 18, and for temporary stays of children under a humanitarian programme — medical treatment, holidays or a school year — their extension, international adoption and the “Vacaciones en paz” programme.

The form is in Spanish and has three pages: two to fill in and one with instructions. EX forms are accepted only in the official version: FoxDocs simply types your data into the fields of the official PDF and does not change its layout.

Who applies and where

  • the holder of the application is the minor, a person with a disability, a former ward, or the accompanying parent, guardian or monitor — one form per person;
  • for a child born in Spain or joining their parents, a parent with a residence permit applies; for a child born in Spain usually within 6 months of birth, otherwise the family reunification procedure applies;
  • a temporary stay under a humanitarian programme (up to 90 days for treatment or holidays, a school year for schooling) is requested by the promoter — a public administration, non-profit association or foundation — 3 to 4 months before the planned date;
  • submitted at the Oficina de Extranjería of the province where the child lives, or online via the Ministry of Territorial Policy e-office (with a digital certificate or through a gestor or lawyer);
  • the fee is paid with form 790 code 052; the tariff item depends on the procedure; pay within 10 working days and check the amount on the ministry website or at the office.

What the form asks for

  • section 1: passport, NIE (if any), surnames, given name, sex, date of birth, place and country of birth, nationality, parents’ names, address in Spain, phone, e-mail, legal representative;
  • section 2: the entity or person with guardianship of the child or promoting the stay — type, relationship, name, ID, address, contacts, representative;
  • sections 3–4: representative filing the application, address for notifications and consent to DEHú e-notifications;
  • section 5: one tick out of 22 items (residence, initial residence, renewal, temporary stay, its extension, other) and, for treatment, the accompanying person or the accompanied minor;
  • place, date, signature and the authority addressed.

Documents usually needed

  • the EX-25 form, original and copy, completed in capital letters;
  • the minor’s full passport, valid for the whole stay;
  • proof of parentage or guardianship and, for programmes, the consent of the parents or guardian;
  • for programmes, the promoter’s documents: NIF, signatory’s authority, report from the child protection authority and a commitment to ensure the child’s return;
  • the receipt for fee 790-052; the exact list depends on the item in section 5 — check the information sheet (hoja informativa) or ask the office.

Frequently asked questions

What is form EX-25 for?

My child was born in Spain — which item do I tick?

Item 5.1 “Menor extranjero acompañado nacido en España (art. 159)”. A parent with a residence permit applies, usually within 6 months of the birth; if that deadline is missed, the family reunification procedure applies.

Does the accompanying parent need a separate form?

Yes. The child and the accompanying parent or guardian each need their own EX-25; the child’s form names the companion and the parent’s form names the child they accompany.

Who applies for a temporary stay under a programme?

The programme promoter — a public administration, non-profit association or foundation. Its details go in section 2, and the application is filed 3 to 4 months before the planned arrival date.

Where do I submit EX-25?

At the Oficina de Extranjería of the province where the child lives, or online via the Ministry of Territorial Policy e-office with a digital certificate or through a professional representative.

How much is the fee and how do I pay?

The fee is paid with form 790 code 052; the tariff item depends on the procedure. Pay within 10 working days of filing and check the current amount on the ministry website or at the office.

How long does a decision take?

Usually one month. If no decision is notified within that time, the application is deemed refused (negative administrative silence).

What happens after the residence permit is granted?

Within one month of notification, apply in person for the TIE card at the police; a minor attends with a parent or guardian.
