What this document is

“Declaração de filiação (menor de idade)” is the Brazilian Federal Police (Polícia Federal, PF) model for minors, from its “Declarações e Formulários” section. The form is bilingual: Portuguese and Spanish.

The table holds the child’s details and the names of the father and mother. The text declares, under Brazilian law, that the parentage details are true and that the birth or marriage certificate or consular certificate cannot be presented because of a vulnerable situation.

Who files it and where

  • the person responsible for the child signs — the form says “Somente o pai ou a mãe”, only the father or the mother;
