What this document is
“Declaração de filiação (maior de idade)” is a Brazilian Federal Police (Polícia Federal, PF) model from its “Declarações e Formulários” section. The form is bilingual: every label is in Portuguese and Spanish.
The adult declarant gives details of their parents (filiação) and declares, under Brazilian law, that the details are true and that they are in a vulnerable situation and unable to present their birth or marriage certificate or a consular certificate.
Who files it and where
- the adult migrant fills it in and signs it personally;
- it is used when the person has no birth or marriage certificate and cannot obtain one;
- the signed declaration is attached to the migration request filed with the PF, together with the other documents;
- for a child there is a separate model, “Declaração de filiação (menor de idade)”.
What the form asks
- full name and date of birth;
- gender (Feminino / Masculino);
- identity card or passport number, and CPF if you have one;
- nationality and country (“País”);
- full names of father and mother;
- place and date; the signature is handwritten.
What to bring
- the signed declaration;
- the document you do have (passport or identity card);
- the other documents on the PF list for your request.
