What this document is
Requerimento de Revisão de Benefício is the INSS form for asking the institute to re-examine a benefit already granted. The editable PDF is published in the “Formulários” section of the INSS website.
According to INSS, a review is requested for example to correct the amount or the contribution time counted, to add, change or remove dependants, or to submit new documents.
Who files it and where
- the INSS benefit holder or their representative;
- the request is made online in Meu INSS (search “Revisão”); if the system is down, by phone on 135; in-person service by appointment through 135;
- INSS states the service is free and replies on average within 30 days; follow it in Meu INSS (“Consultar Pedidos”).
What the form asks for
- applicant’s name, address (street, number, bairro, city, UF), phone and e-mail;
- the number of the benefit (NB) to be reviewed;
- reasons for the review — up to 20 lines;
- place, date and signature of the applicant or representative.
What to have at hand
- identity document (RG, CIN, CNH or CTPS), CPF and the benefit number — the list INSS gives;
- documents supporting your point: work booklet, CNIS statement, certificates;
- for a representative — their ID, CPF and a power of attorney (INSS model or notarial) or proof of legal representation.
