What this document is

คำร้องขอจดทะเบียนคนเกิด is the application used to register, under Thai civil registration, the birth of a child born abroad. The registration office printed on this blank is the Royal Thai Embassy in Oslo (สถานเอกอัครราชทูต ณ กรุงออสโล).

The form is one page, in Thai. It covers the child, the house registration (ทะเบียนบ้าน) the child is to be added to, the mother, the father and the informant.   • the Thai house registration (house no., Moo, tambon, amphoe, province) the child is added to, if any;
  • mother and father: name in Thai and English, maiden name of the mother, Thai ID number, age at the child's birth, nationality, province and country of birth, Thai registered address, address abroad, phone;
  • informant: name, Thai ID number, age, address, relation to the child, and the authority that issued the local birth certificate;
  • signature — by hand. The parts for the receiving officer and the registrar are completed by the embassy.

What to attach

  • the local birth certificate issued in the country of birth;
  • passports and Thai ID cards of the parents;
  • the Thai house registration book (ทะเบียนบ้าน), if the child is to be added to it;
  • the parents' marriage certificate, if the parents are married.
