What this document is

"แบบถ่ายทอดความรู้/รายงานการฝึกสอนพนักงานคนไทย" (The form of knowledge transfer / The report for training Thai employees) is a one-page form of the Department of Employment (กรมการจัดหางาน), Ministry of Labour of Thailand, printed in Thai and English.

A foreign national working under a work permit uses it to report which Thai employees they trained and what was covered: theory, practice and results. The report is signed by the foreign national and by the Thai employees.

Who files it and where

  • a foreign work permit holder whose permit carries a condition to transfer knowledge to or train Thai employees (the "เงื่อนไข / Condition" line);
  • usually together with the employer's work permit documents;
  • at the employment office (สำนักงานจัดหางาน) handling the work permit — check with the office when and with what the report should be filed.

What the form asks for

  • the foreign national's name, nationality, position and work permit number;
  • the company name and the condition the report is made for;
  • up to three Thai trainees: name, education, position;
  • details of the training — theory, practice and results (8 lines on the form; a longer text continues on the attachment sheet);
  • names under the signatures of the foreign national and two Thai employees, and the date (Buddhist Era year, พ.ศ.).

Documents to attach

The form allows an extra page to be attached if the training description does not fit. It does not list any other attachments — check the required documents, deadlines and any fee with the employment office.

Who fills in this report?

The foreign work permit holder who trains Thai employees. It is signed by them and by the Thai employees.

When is it needed?

When the employment office made knowledge transfer or training of Thai employees a condition of the work permit. The form does not say when exactly to file it — check with the employment office.

Where is it filed?

At the employment office (สำนักงานจัดหางาน) that handles your work permit. Check the filing procedure there.

In which language is it filled in?

The form is bilingual, Thai and English. The foreign national fills it in English; names are written as in the passport or the English line of the Thai ID card.

How many employees can be listed?

The table has three rows and there are two signature lines for Thai employees. If more were trained, ask the office whether a list can be attached.

What if the training description does not fit?

The form explicitly allows continuing on a separate page. If the description is longer than eight lines, the rest is printed automatically on an attachment sheet (page 2 of the PDF) with the applicant’s name and a place for the signature. If everything fits on page 1, the attachment need not be submitted.
