What this document is

WP.61 (บต.61) is a foreigner’s application to the Department of Employment (DOE), Ministry of Labour, for a substitute work permit (ใบแทนใบอนุญาตทำงาน). It is used when the original work permit has been lost, destroyed or substantially damaged.

The form is bilingual (Thai and English) and has 2 pages: page 1 — details of the foreigner, the reason for replacement, the document checklist confirmation, signature and a personal data consent; page 2 — the checklist of attachments and a section for official use only.

Who files it and where

  • a foreigner holding a Thai work permit that has been lost, destroyed or substantially damaged;
  • the form is submitted to the Department of Employment — in Bangkok the DOE office for foreign workers or an area employment office, in other provinces the provincial employment office; check with DOE which office handles your permit;

What the form asks for

  • title, name, nationality, date of birth, age;
  • identity document (type and number) or a note that there is none;
  • name of the employer, address in Thailand, telephone, province where the work permit was issued;
  • what happened to the permit — destroyed (cause and date), lost (date) or substantially damaged (cause and date);
  • whether the attached documents are complete, date and signature;
  • two consent questions on the use of personal data by the DOE.

Documents to attach (checklist on page 2)

  • if you have an identity document — a copy of the passport, or of a travel document in lieu of passport, or of the alien registration book and certificate of residence, or of a non-Thai ID card, or other proof;
  • proof that the permit was damaged, destroyed or lost — e.g. the damaged permit or its photo; if lost, a police report or testimony memorandum;
  • two 3×4 cm photos taken within the last six months, facing the camera, without a hat or dark glasses.

The office may ask for additional documents — check with it before submitting.
