What this document is
WP.48 (Work Permit Application Form on behalf of foreign nationals of Cambodia, Lao PDR, or Myanmar) is an application to the Thai Department of Employment (Ministry of Labour). The employer files it on behalf of a worker who is a national of Cambodia, Lao PDR or Myanmar to obtain a work permit under the procedure set by the Cabinet Resolution of 29 December 2020.
The form also covers workers without documents: it has a box "does not have the required documents and proofs". Whether this procedure is still open and for which workers — check with the Department of Employment.
Who files it and where
- the employer (an individual or a company) files and signs it — the form names the employer as the applicant;
- the worker must be a national of Cambodia, Lao PDR or Myanmar;
- it is filed at the Department of Employment office for the place of work (an area office in Bangkok, a provincial office elsewhere); check the exact office, procedure, fees and processing time with the office.
What the form asks for
- employer: name, address, telephone, fax;
- worker: title, name, nationality, date of birth, age; whether there is an identity document and its details — number, issuing authority and country, date of issue and expiry;
- type and nature of work, address of the place of work, requested duration (years, months, days);
- list of attached documents and date of signature.
Documents to attach (as listed on the form)
- if the worker has a document — a copy of one of them: passport, travel document in lieu of passport, certificate of identity, border pass, pink card, work permit or home-country ID (expired documents are accepted);
- a full-face photograph of the worker without a hat;
- a medical certificate from a licensed doctor: no mental disorder and no prohibited diseases, including COVID-19;
- employer documents: for an individual — a copy of the ID card, passport or certificate of residence and, if any, a business licence; for a company — a business licence or registration certificate showing the type of business;
- if the worker will be a shop assistant — a copy of the commercial registration or licence, the income tax return and the social security contribution list or a Thai employment certification.
The current list, fees and processing times depend on the office — check with the Department of Employment.
Which language to use
The form is in Thai with English captions. FoxDocs fills it in Latin capital letters: the worker's name as in their document, the employer's name and addresses in transliteration. Ask the office in advance whether it accepts Latin script or needs a Thai version.
