What this document is

The Consent Form for Sponsorship of Dependent is a one-page declaration published by the Board of Investment (BOI) on the Long-Term Resident (LTR) visa portal. In it the main LTR applicant declares sponsorship of and financial responsibility for a dependent family member — a spouse, child, parent or other family member — for the time the dependent stays in Thailand.

The form is part of the dependent's LTR application. It does not replace the visa application itself or the documents that prove the family relationship.

What the main applicant confirms (as printed on the form)

  • the relationship to the dependent, according to the documents provided, is genuine and stated in good faith;
  • the dependent will live together with the main applicant as part of the family;
  • the main applicant bears the financial support, accommodation, health and other necessary expenses, including medical care, so that the dependent does not need Thai social welfare or third-party support (insurance companies excepted);
  • if the person stops being a dependent — a child turns 20, or there is a separation, divorce or dispute within the family — the main applicant will notify the officer at once so that the dependent's visa is cancelled; otherwise the main visa may also be terminated;
  • the dependent will comply with Thai immigration law and other Thai laws and regulations.

What the form asks for

  • the date;
  • the main applicant's full name, citizenship and LTR application number;
  • the dependent's full name;
  • two signatures with dates: the main applicant's and the dependent's (for a child under 20, a parent signs on the child's behalf).

What to prepare

  • the main applicant's passport and LTR application number;
  • the dependent's passport;
  • documents proving the relationship (for example, a marriage or birth certificate) — the form refers to them as “the document provided”.
