What this document is

The “Application for Qualification Endorsement for Dependent” is a form of the Board of Investment of Thailand (BOI) for the Long-Term Resident (LTR) visa programme. It is filled in for a dependent — the spouse or a child — of the main LTR applicant or holder, so that BOI can confirm the dependent's qualification for the LTR visa.

The official file has five pages: the application itself (Parts A, B, F, G, H, I, J), the employment certificate form WP.46 and the list of required documents.

What the form asks for

  • Part A — full name as in the passport, title, nationality, date and place of birth, sex, marital status, passport details, home address, address in Thailand, phone and e-mail, relationship with the main LTR holder (spouse or child) and a contact person in Thailand;
  • Part B — means of medical expense: health insurance, social security benefits or a bank deposit (only one option);
  • Parts F and G — only if the dependent plans to work or already works in Thailand: company information and job details;
  • Part H — travel information: visa, port of entry, means of transport, arrival date, permitted stay, TM.6 number;
  • Parts I and J — the WP.46 form signed by the employer and the place to collect the digital work permit (for those who work).

Required documents (as listed on the form)

  • copy of a valid passport (first page and all pages with Thai visa stamps); the passport must be valid for more than 6 months when the visa is collected;
  • photograph 3×4 cm;
  • evidence of family relationship, e.g. a marriage or birth certificate;
  • health insurance with hospitalisation cover of no less than 50,000 USD (may be submitted later, before the notification letter is issued), or evidence of social security benefits, or evidence of a bank deposit;
  • for those who work: employment certificate WP.46 and, if any, a certificate of employment, a copy of the work permit and of its cancellation;
  • a criminal record check from the country of nationality or residence may be requested case by case.

Note: the application page states a deposit of no less than 100,000 USD, while the list of required documents in the same file states no less than 25,000 USD held for at least 12 months. Check the current requirement with BOI before relying on this option.

Where and how it is submitted

The form belongs to the BOI LTR programme; documents are uploaded together with the application (the form itself asks to print WP.46, have it signed and upload it). The exact submission channel, fees and processing times are not stated on the form — check them on the BOI LTR portal ltr.boi.go.th or with the BOI One-Stop Service.
