What this document is
Solicitud de revisión de grado de dependencia (form F89176) is filed with the Conselleria de Servicios Sociales, Familia e Infancia of the Generalitat Valenciana. It asks for a review of a dependency degree (grado de dependencia) that has already been recognised by a resolution.
One of three grounds is chosen: deterioration (agravamiento), improvement (mejoría), or an error in the diagnosis or in applying the official BVD assessment scale.
Who files it and where
- the person who already has a dependency degree resolution, or their representative or support person;
- without a degree resolution a review is not possible, as the form instructions state; the initial dependency application comes first;
- the form is addressed to the town council: the header reads “Ayuntamiento de …”; check filing options with the council or on the Generalitat website.
What the form asks
- the reason for the review and the town council;
- applicant details: surnames, name, DNI/NIE, SIP health card number, address, phone, e-mail;
- health centre and social services centre, living in a care home, disability and its type, “gran incapacidad” pension, schooling under 23;
- a separate address for notifications, if any, and whether there is a representative;
- consent to tax data checks and objections to other checks.
What to bring
- reports showing the change in health or circumstances that justify the review (section D of the form);
- the form “Identificación de personas representantes o de apoyo” if there is a representative and it has not been submitted before;
- a copy of the DNI/NIE of the applicant and the representative — the form recommends attaching it.
