What this document is
C-157 (“Asistencia sanitaria para migrantes en estancia temporal en España”) is the official form of Spain’s National Social Security Institute (INSS). Spaniards who live abroad use it to obtain public healthcare during a temporary stay in Spain, for themselves and for the relatives travelling with them.
The stay may not exceed three months; it can be extended by another three months on request.
Who files it and where
- case A — a Spaniard by origin (español de origen) living abroad who works or receives a pension there, plus accompanying relatives;
- case B — a Spaniard by origin who is a Spanish Social Security pensioner, or a person with a Convenio Especial, living abroad — for the relatives travelling with them;
- relatives — spouse or registered partner (pareja de hecho) and dependent children under 26 (over 26 with a disability of 65 % or more); they must travel with the applicant;
- the worker or pensioner always files: online at the Social Security e-office (certificate or Cl@ve), via run.gob.es, or a signed copy by post or at an INSS centre (CAISS) by appointment.
What the form asks
- applicant: surnames, given name, date of birth, sex, marital status, DNI/NIE/passport, country of residence, address abroad, phones, e-mail;
- period of stay in Spain — from and until;
- which documents are enclosed (tick boxes);
- up to six relatives: surnames, given name, date of birth, sex, marital status, DNI/NIE, Social Security number, passport, parents’ names, nationality;
- statements, two consents, place and date of signing, INSS province.
What to bring
- a full birth certificate (certificación literal de nacimiento) proving Spanish nationality of origin — only the first time;
- a certificate of registration in the Register of Spaniards Resident Abroad (PERE);
- in case A, proof that you work or receive a pension in your country of residence;
- if you come from Andorra, Chile, Morocco, Peru, Tunisia, Canada, an EU/EEA country, Switzerland or the UK (case A) — a certificate from that country’s social-security institution that your health cover cannot be exported;
- for relatives — the family book or marriage certificate, partnership registration for a pareja de hecho; for children over 26 — the disability certificate.
