What this document is
851F3 (“Solicitud de Cancelación de inscripción en el Registro de Uniones de Hecho (mediante representante con poder notarial especial)”) is the official form with which a representative holding a special notarial power of attorney asks the Madrid region’s Register of Partnerships to cancel a couple’s registration. Two pages to fill in and two pages on data protection, in Spanish.
Under art. 6 of Madrid Law 11/2001 a partnership ends by mutual agreement; by decision of one member notified to the other; by the death or declaration of death of one member; after more than six months of living apart; by the marriage of one member; or when one of the members is no longer registered (empadronado) in a municipality of the Madrid region (this ground was added by Law 11/2022 and applies to couples registered from 23.12.2022; the form words it as “neither member is registered”). One member alone may request the cancellation — the register then informs the other.

What the form asks for
- for each member: NIF/NIE, passport, both surnames, given name, address (street type and name, number, floor, door, postcode, town, province), fax, e-mail, phones;
- the representative: NIF/NIE, surnames, given name, e-mail, fax, phones;
- how to be notified: electronically (only for users registered in the Comunidad de Madrid notification system) or by registered mail — then a postal address;
- attachments: the notarial power of attorney and the fee receipt (form 030);
- the reason for cancellation and its supporting document, place and date of signing.
What is usually attached
- the special notarial power of attorney to cancel the registration;
- the receipt for the fee paid with form 030;
- for a decision of one member — the notice to the other (registered letter, telegram or burofax);
- for a death — the death certificate; for a marriage — the marriage certificate;
- if the members are no longer registered in the Madrid region — a registration certificate from another region or proof of deregistration. Confirm the exact list for your case with the register.
