What this document is
EX-16 (“Solicitud de cédula de inscripción o título de viaje”) is the official form for undocumented foreign nationals. The cédula de inscripción (art. 210 RD 1155/2024) is an identity document Spain issues to someone whom no country can or will document; it is renewed every year. The título de viaje (art. 211) is a travel document for someone who must leave Spain and cannot obtain a passport of their own; it may include return to Spain or not.

Who files it and where
- a foreign national whom no country will document — they must prove the consulate does not document them (under art. 210.3 by a notarial deed; for minors and young people formerly in child-protection care, a report from that authority is used instead);
- a foreign national who must leave Spain and cannot obtain their own passport — for a título de viaje;
- filed in person and in writing — at a National Police station (Comisaría) or an Oficina de Extranjería;
- fee: form 790 code 012; per the National Police fee table as of 2026-10: cédula de inscripción — €3.27, título de viaje — €27.08, and a separate line “autorización de inscripción de indocumentados” — €21.87; check which amount applies to you and whether it is current on sede.policia.gob.es;
- decision within 3 months; with no answer the application is deemed refused (silencio negativo).
What the form asks
- personal details: passport number (if any), NIE, surnames, given name, sex (H/M/X), date of birth (dd/mm/yyyy), place and country of birth, nationality, marital status, parents’ names;
- address in Spain, mobile phone and e-mail; legal representative, if any;
- representative filing the application (section 2), address for notifications (section 3) and consent to DEHú electronic notifications;
- section 4.1: cédula (first issue or renewal; special case — unaccompanied minor, former ward, exceptional circumstances) or título de viaje (with or without return, destination country);
- section 4.2: grounds — humanitarian reasons, public interest or commitments undertaken by Spain;
- place, date, signature and, at the bottom, the authority the application is addressed to.
What people usually bring
- form EX-16, original and copy (as the form’s instructions require);
- receipt for the 790-012 fee;
- any documents pointing to your identity, origin and nationality, even expired ones (art. 210.3);
- proof that the consulate does not document you and documents supporting the grounds in section 4.2 — confirm the exact list with your office.
