What this document is
The “Documento de designación de representante” is a template from Spain’s Ministry of Inclusion, Social Security and Migration, published under “Modelos de comunicaciones”. With it a foreign national appoints a representative who files an immigration application on their behalf, signs documents and takes part in the procedure — except for steps that require the person to appear in person. The form cites articles 5 and 66 of Law 39/2015, article 197 of RD 1155/2024 and article 11 of RD 240/2007.
The ministry describes these templates as indicative, not mandatory: representation may also be proven by other lawful means. A ready template, however, is understood by every office. FoxDocs writes your data into the fields of the ministry PDF and does not change its text.
Who files it and where
- a foreign national whose EX application is filed by someone else: a gestor, lawyer, graduado social, employer or friend;
- under article 5 of Law 39/2015 any person with legal capacity may act as a representative, and an organisation may do so if its statutes provide for it; an application on someone else’s behalf requires proven representation;
- the foreign national signs the document, and the representative attaches it to the EX application — on paper at the office or as an upload when filing online;
- in the EX application itself the representative’s details go into the section “Datos del representante a efectos de presentación de la solicitud” (its number differs between EX forms).
What the form asks
- the foreign national’s details: given name, two surnames, nationality, NIE (if any), passport number, date of birth (dd/mm/yyyy), town and country of birth, father’s and mother’s names, marital status (S/C/V/D/Sp);
- the foreign national’s address in Spain, phone and e-mail;
- which application the representative is to file (“solicitud de …”);
- the representative’s details: DNI/NIF/NIE, firm name (if any), given name and surnames, address in Spain, phone and e-mail;
- consent to electronic notifications via DEHú (optional);
- place and date of signing, the foreign national’s signature.
What to bring
- the completed template — the form states that an original and a copy are submitted;
- a copy of the foreign national’s passport or NIE and of the representative’s DNI or NIE — check the exact list with the office;
- the EX application itself and the documents your procedure requires.
How to fill it in with FoxDocs
- Tell the chat in any language who you are, who your representative is and which application they will file.
- The assistant fills in the fields in capital letters, splits the date of birth into dd/mm/yyyy boxes and ticks the marital status.
- Download the PDF, check it, sign it by hand and give it to your representative together with a copy.
